Initial RN Assessment
This assessment is conducted by a registered nurse and is required prior to a resident receiving any home care services. This does not include housekeeping.
Annual RN Reassessment
Health Monitoring (every 3 months)
After an initial assessment, residents who do not obtain regularly scheduled services receive quarterly health monitoring from our nurses.
Licensed Nurse
Services may include, but are not limited to:
· Medication set-up and management
· Health monitoring visits every 90 days
· General health assessments
· Medication review
· Consultation with health care professionals
· Health care counseling and advocacy
· Weekly/monthly injections
· Schedule in-home lab visits
· Wound care
Medication Set-Ups
(up to 30 minutes)
Complex Medication Set-Ups
(up to one hour)
Medication Change and New Order
Resident Assistant Services
Resident assistants provide a variety of personal care services on a scheduled basis. These services may include, but are not limited to:
· Medication administration or reminders
· Escort to meals and activities
· Bathing assistance
· Dressing assistance
· Mobility or exercise assistance
· Hair, nail, or skin care
· Safety checks
Escort Packages
Resident assistants are able to provide escorts (either walking alongside a resident or pushing them in a wheelchair).
· One escort daily (both ways)
· Two escorts daily (both ways)
· Three escorts daily (both ways)
